Position Summary
The Case Manager, in collaboration with the physician,providesindividual program management for each patient toensure the patient’s progression through the continuum of care in a manner that achieves the desired clinical andfinancial outcomes. Monitors and manages clinical and financial coordination of treatmentplanof assigned patientsto ensuretimely, cost-effective, individualized service delivery. Works with rehabilitation patients with variousdisabilities including, but not limitedto:spinal cord injury, brain injury, cerebrovascular accident, amputation,neurologic disorders, orthopedicconditions, and arthritis. Coordinates length of stay management within Medicare(CMS) guidelines and 60% compliance threshold.
Minimum Qualifications
* Current state licensure as a Registered Nurse, Licensed Social Worker, PT, OT, SLP.
* Minimum of 3 years healthcare and clinical experience in a hospital setting.
Desired Qualifications
* 1 yearmedical rehabilitation experience.
* Certification in case management preferred.

* Excellent communication, negotiation, and conflict resolution skillsrequired.
* Knowledge of reimbursement systems preferred.
* Excellent verbal and written communication skills.
* Strong organizational, time management and prioritization skills.
* Strong analytical and critical thinking skills
* Detail-oriented, able to meet stricttime frames
