Significant adverse event review support - mental health services and addictions

The Role

The Significant Adverse Event Review (SAER) Support Team has been supporting SAERs commissioned within Mental Health Services since 2022 and is expanding to support additional areas and specialities. The post holder will play a key role across Mental Health Services, ensuring that clinical care and services are high quality, safe, effective and person‑centred.


Key Responsibilities

* Provide a high‑quality administrative service to support the SAER process and review teams.
* Provide and receive complex, multifactorial clinical quality information; produce written and verbal reports for key stakeholders.
* Work within defined parameters, exercising significant discretion and independent judgment.
* Update patients and families on SAER progress, completion dates and delays.
* Respond to queries from patients, families and staff regarding SAER policy, involving review and management teams as appropriate.
* Obtain and communicate patient/family feedback on the SAER process to the Clinical Risk Team.
* Maintain and update information on SAERs in relation to review progress.
* Collate relevant information from patient electronic records as advised by the review team.
* Request copies of paper patient records where appropriate from medical records or the patient’s GP.
* Maintain SAER file of all documentation.
* Ensure confidentiality of personal data and patient/client files, applying appropriate security systems in line with Data Protection and Information Security Policies.


Essential Criteria

* Excellent communication and organisational skills.
* Ability to work independently and manage time and tasks effectively.
* Experience of incident investigation and root cause analysis.
* Experience of extracting and summarising complex data from information systems.
* Experience of minute taking and transcribing meetings.
* Working knowledge of Office 365 (Word, Excel, Teams, Outlook).


Desirable Criteria

* Knowledge of patient information systems such as EMIS and Clinical Portal.
* Experience of undertaking Significant Adverse Event Reviews (SAERs).
* Experience of having ‘difficult conversations’ with staff and the public.


Location and Working Pattern

* Location: Brand Street Resource Centre
* Contract: Permanent
* Hours: Full‑time, 36 hours per week (Monday to Friday, 09:00–17:00)
