Role Overview
NHS Greater Glasgow and Clyde is delighted to offer an opportunity for a Community Staff Nurse to join our District Nursing teams within Primary Care Services. The role involves delivering high‑quality, person‑centred nursing care to patients in their own homes and community settings. Candidates will work both independently and as part of a multidisciplinary team, caring for individuals with complex and long‑term needs.
Duration, Location, and Working Pattern
* Based between Possilpark Health & Care Centre, G22 5AP and Centre for Health & Care Plean Street, G14 0YH
* Full Time, Permanent
* Working Monday to Sunday, fully flexible
Key Responsibilities
* Assess, plan, implement, and evaluate programmes of care in partnership with patients, families, and carers
* Deliver skilled nursing interventions to patients with complex and long‑term conditions in the community
* Manage and prioritise own workload effectively to respond to planned and unplanned care needs
* Maintain accurate up‑to‑date clinical records in line with professional and organisational standards
* Provide clinical leadership and act as a role model for junior staff and students
* Work collaboratively with multidisciplinary and multi‑agency colleagues to coordinate care and support discharge planning
* Promote patient safety and well‑being through effective risk assessment, incident reporting, and adherence to policies
Qualifications and Experience
* Registered Nurse with current NMC registration and a live NMC UK pin
* Relevant post‑registration experience
* Evidence of continuing professional development and commitment to lifelong learning
* Ability to work independently and autonomously within a community setting
* Strong communication, interpersonal, and organisational skills
* Experience of delivering person‑centred care to patients with complex needs
* Ability to work effectively as part of a multidisciplinary team with a flexible and problem‑solving approach
