This is a Relationship Role – Practice Facilitator
Are you a relationship driven person experienced in building and maintaining strong positive relationships with stakeholders. Our role is centred around relationships, regular interaction and supporting our primary and community care health professionals in general practice. This is an exciting opportunity to develop or expand your knowledge of primary and community care, and to support the future improvements in primary care service delivery for our Northland population.
In this role you will engage with General Practices and guide an improved understanding and application of equity across primary health care services. Underpinned by the National Pae Ora Healthy futures framework, our Mahitahi Papa Tikanga framework, and Equity for Whanau agreement, this role will support capability for primary healthcare professionals in embracing an equity focus and approach.
About Mahitahi Hauora
A NFP Charitable Trust, Mahitahi Hauora PHE works collaboratively with community, primary healthcare providers and key partners to support general practices, Māori health providers and social sector entities in achieving better health outcomes for our Tai Tokerau population.

The Position
This Facilitator position is based in Whangarei. Working alongside your General Practice Facilitator colleagues you will support practices and providers across primary healthcare programmes, improvement methodologies and equity strategies. This is a varied role that provides opportunity for growth in skill set, relationship management and quality and service improvement initiatives.
You will need to be passionate and enthusiastic in your approach, have a strong desire to support our primary healthcare and community environment and be prepared to be flexible and innovative.
This role would be ideally suited to someone who has a current or previous background in health, and who may be looking for something a little different or moving away from direct clinical work but still want to be involved in the health or community sector. Alternatively, it may suit someone with a strong general practice administration background. If this sounds like you, and you have what is needed to be considered for this role, then please apply.
While not essential, an understanding or experience in change management strategies, improvement methodology, practice management systems and analytics would be useful. However, to support your success in this role, you will undertake broad orientation and training.
The following experience and skills to match would be ideal, however applications deemed relative to our requirements will be considered.
* A minimum of 3 years’ experience in the primary/community healthcare sector, or evidence of a strong understanding of this sector.
* An intermediate to advanced level of computer literacy across MS Office 365, MSTeams
* An understanding of and commitment to Te Tiriti o Waitangi
* A current full NZ drivers licence (un-encumbered)
* Evidence of:
o Developing and manging relationships with health professionals
o Understanding and implementing change in an organisation
o A confidence in communication both spoken and written
We will also be interested in your understanding and knowledge of equitable health outcomes.
You must be a NZ Citizen or Permanent resident.
