This isn’t your average case management role — with the rollout of the Support at Home Program, you’ll have the chance to lead with impact, empower clients, and help older Australians live life their way.
And yes… you’ll get a company car to travel around Bundall, Tweed Heads, and the beautiful surrounding suburbs.
About the Role
As a Care Partner, you’ll be the go-to person for coordinating care that truly puts people first. You’ll work side by side with clients and families to navigate services like allied health, nursing, and home support.
Think of yourself as the connector, advocate, and coach — helping clients stay independent while living life on their own terms.
What You’ll Be Doing
* Create and deliver personalised care plans
* Support clients in setting meaningful goals and budgets
* Manage your own caseload with regular reviews and assessments
* Build strong, trusting relationships with clients and families
* Coordinate services across health, home, and community care
About You
You’ll shine in this role if you’re organised, empathetic, and passionate about improving lives.
Must-haves:
* Experience in aged care case management (HCP/CHSP preferred)
* Background in community or aged care service coordination
* Great communication and planning skills
* Ability to work independently and as part of a team
* Able to obtain a Police Check & NDIS Worker Screening
Bonus points if you have:
* Nursing qualifications (RN or EN)
